making my way?

i am in the middle of week 6/14 and starting to gain a bit more responsibility. i had three patients, one went out acute due to a mental change, one was supposed to be discharged but was unable because mass health will not cover some equipment she needs for wound healing and therefore staying with us would be better for her healing, so now i have two. i continue to struggle the most with treatment planning. i often think, how is this treatment session occupation based? typical treatment sessions can consist of going through the toileting routine- coaching the patient on proper body mechanics when ambulating to the toilet, assisting them if need be in the transfer, hygiene, clothes management; the bathing routine- either sink level or in the shower with adaptive equipment- coaching similar to the toileting on proper body mechanics, taking rest breaks, and assistance as needed; dressing- can occur after the bathing/showering- this one can be difficult at times if the patient tends to stick to wearing the johnny, as real clothes can be more realistic for assessing and doing; functional mobility and transfers or sit-to-stands; therapeutic exercise to help strengthen the upper body; and possible light meal prep and home management (laundry). sooo, my biggest concern when planning treatment sessions is to figure out a way to make it occupation based and meaningful to the client.
