Mr. Tony Update

I realize that I never posted further on Mr. Tony as I saw him for maybe two treatment sessions and then I was away on vacation to New York. Here is an update:

As I had mentioned, Mr. T had his right leg amputated below the knee several years ago and has been using a prosthesis and his left leg was amputated possibly 5 or so months ago but came to us with an infection at the wound site. so he had been living alone at wheelchair level and getting around independently. He definitely showed areas where he could benefit from Occupational Therapy (safety awareness, BUE strength, problem solving skills, standing balance/tolerance for lower body dressing/bathing, and toileting routing). However, felt that he didn't need to participate in therapy and thus his services have been discharged.

Throughout the couple of treatment sessions I saw him we worked on upper body strength training (mainly triceps and shoulders for wheelchair mobility and sit-to-stands and functional transfers), functional transfers, lower body dressing and standing balance/tolerance activities. He was definitely making improvements, but also should room for further improvement to reach his prior level of function. But it is his right to refuse treatment and there is nothing worse then trying to convince a patient to participate when they don't want to.

Is it worse to convince a pt to participate or knowingly send them home unsafe without trying to address deficient skills?

Hi Cheryl,

That is a good question. I always try my hardest to convince the patient to continue to participate in OT and I am generally quite good about it. However, this patient was so adamant that he didn't need OT and we have to respect his rights of course and we cannot force him. So, even though it is a tough situation sending someone home who may not be safe, it is the only thing that can be done in this situation. And, all we can do is offer our professional opinion and advice.
